FRACAS stands for "Failure Reporting, Analysis, and Corrective Action System." It is a structured methodology and process used in various industries, including manufacturing, aerospace, defense, automotive, and healthcare, to systematically identify, analyze, and address failures, defects, and non-conformances in products, processes, or systems. FRACAS involves capturing data on failures and incidents reported by users, customers, or internal sources, such as quality control inspections, testing, or maintenance activities. The collected data is then analyzed to identify patterns, trends, and root causes of failures, using techniques such as fault tree analysis, Pareto analysis, and statistical process control. Based on the analysis findings, corrective and preventive actions (CAPAs) are developed and implemented to address the underlying causes of failures and prevent recurrence in the future. These actions may include design changes, process improvements, supplier quality management, training, and procedural changes. FRACAS also involves tracking and monitoring the effectiveness of CAPAs over time, through follow-up inspections, audits, and performance metrics, to ensure continuous improvement and compliance with quality standards and regulatory requirements.
